Retina Consultants of Houston is the largest retina-only ophthalmology practice in Southeast Texas, with 9 physicians and 10 convenient locations in Houston and surrounding areas, and one of the largest clinical retinal research centers in the country. To refer a patient to Retina Consultants of Houston please contact one of our oces with the patient’s information. To allow us to provide the best care for your patients please fill out a Consultation Request Form (PDF) describing the reason for the referral and a summary of any diagnostic tests that have been performed. The Consultation Request Form will be given to the patients to bring with them to their examination.

How to Refer a Patient

To assist in making your patients care with us a smooth transition , we have a consultation request forms available, as well as brochures, new patients packets and business cards.
